The spokesman for Shatin Hospital (SH) announces a sentinel event today (November 25):
A 51-year-old male patient admitted to the psychiatric ward was found unconscious by staff in the toilet of the ward at around 10.40am.The patient's neck was coiled around with clothing. Healthcare staff performed resuscitation and informed patient's family immediately. The patient was transferred by ambulance to Prince of Wales Hospital for further treatment. The patient was certified dead at 11.42am.
The hospital reported the case to the Police and referred to the Coroner for follow-up.
SH was deeply saddened by the incident. The hospital expressed condolences and will render necessary assistance to the family members. The incident has been reported to the Hospital Authority Head Office via the Advance Incident Reporting System. A root cause analysis investigation will be conducted to look into the incident and the report will be submitted in eight weeks.
